For students who have had a change in either name or address. Change in name must be accompanied with copies of legal documentation.
For students to add courses to their schedule. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.

Undergraduate degree seeking students who previously attended Webster University but have been out for a calendar year or longer must complete this form and an advising appointment before they are able to register.
Complete this online form and provide required supporting documentation.
Students who wish to use financial aid funds to pay for their textbooks must apply online. This program is limited to St. Louis area campuses (Westport, Winghaven, Downtown and Webster Groves) and online students.
For students to drop courses from their schedule. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.
For students to withdraw from a course after the drop period has ended. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.
Students should use this form to request permission to take courses at a non-Webster institution for Webster credit.
The Office of Student Affairs processes requests from students for tuition waiver for undergraduate courses. Tuition refunds are contingent on the drop or withdrawal date. It is the undergraduate student's responsibility to file a drop or withdrawal form with the Registrar's Office.
