All residents are required to submit a completed Student Health Packet and carry health insurance (either your own or the Webster University Student Insurance Plan).

To avoid any delay checking into the residence halls or apartments during move-in day, your Student Health Packet must be complete. A complete Student Health Packet consists of the following forms:

• Emergency Information and Medical Release
• Student Health Insurance Waiver and Medical History
• Physician Assessment
• A copy of both sides of your medical insurance card

Residents who do not provide a complete Student Health Packet by the deadlines stated below will automatically:

• Be charged for the Webster University Student Insurance Plan;
• Be delayed at move-in; and,
• Have a hold placed on your student account inhibiting your ability to register for class.

Completed Health Packet Due Dates:

Fall Semester: Due by July 1

Spring Semester: Due by December 1

Summer Term: Due by May 1
