Requesting an Absence or Disability 

Instructions

Webster University is happy to announce that effective May 1, 2020 we are  transferring absences for leave and disability to The Prudential Insurance Company of America. Prudential will be responsible for reporting the following types of absences:

  • Short Term Disability (STD)
  • Family Medical Leave (FML)
  • Long Term Disability (LTD)
  • Military Duty
  • Work Related Injury

If employee’s will be absent more than 3 days and are under a physician’s care, are hospitalized for any amount of time, caring for an ill or injured family member, pregnant, will be absent periodically due to a chronic condition/pregnancy, caring for a newborn or recently adopted child or new foster child, or due to a work related injury. Employee’s will call Prudential at 877-FOR-PRU1 (877-367-7781) Monday through Friday 8:00 AM-11:00PM EST or you can log into www.prudential.com/mybenefits and click on “Claims and Absence” and then “File a Claim/Report an Absence.” Make a copy of the authorization, sign and date it and present to your doctor to file with Prudential. Anytime you are talking with claims manager from Prudential employee’s will need to provide the following information:

  • Company Name: Webster University
  • Company Control Number: 52654
  • Employee ID/Social Security Number
  • Reason for Absence
  • First Date of Absence
  • Work Schedule
  • Date you expect to Return
  • If you absence is related to a work injury
  • If employee is caring for a family member

Employee’s will notify Prudential if they have updated information, unable to return to work on the planned date, are returning to work , want to report a delivery date, want to report intermittent time out of the office, or if forms are needed. Prudential is available to offer Webster employee’s personalized support every step of the way! *Employee Record Forms (ERF) will still be required to be processes interdepartmentally and sent to HR for processing.
