  • poor direction and goal setting (76%)
  • failure to adapt leadership approach  (76%)

  • under or over-supervision (27%)
  • lack of management skills (14%)
  • lack of or inappropriate support (12%)
  • lack of accountability (5%)

In another study about why CEOs are fired, the researchers found:

  • 31% mismanage change,
  • 28% ignore or alienate customers,
  • 27% tolerate low performers,
  • 23% deny reality, and
  • 22% are too much talk and not enough action (LeadsershipIQ.com, 2005)
