ITM 5000 STUDENT SURVEY


This survey is designed to help me evaluate the educational and occupational background of the each class. Since class rosters are not available until the first night of class, this allows me (and you) to save time on the first night of class.

1. At the bottom of the page, there are 9 questions for you to answer.  
2. Go to your e-mail program and click on NEW MESSAGE. Then cut-and-paste these 9 questions into the body of the e-mail message.
3. In the TO box, enter kozlowski@webster.edu.
4. In the SUBJECT line of the message enter ITM 5000 STUDENT SURVEY.
5. I recommend that you also send a copy to yourself using the CC: feature of the e-mail message. Retain this copy until you see that I have acknowledged receipt of your submission by posting your name on the ROSTER page (in the REPORTS section of the ITM 5000 web site).

I post your test and homework grades on the web site. To insure privacy, you will need to submit a PIN number for me to use to identify your grades. The PIN must consist of ONE LETTER and 4-NUMERIC DIGITS. EXAMPLE: S-1234. 

To designate the semester you will be attending, select SPRING 1, SPRING 2, or FALL 1.

To designate your Webster major, use the following codes:
ITM - Info. Technology Mgmt.
HRDV – Human Resources
INTL – International Relations
MBA – Masters of Business Administration
MNGT - Management
PROC – Procurement

Webster’s computer lab policy prefers that students sit in the same seat each week so that if there are any technical or physical problems, they can talk to the student who was the last one to use that PC. Therefore, you should go to the SEATING CHART web page for your specific semester and select which seat you want. Although I will periodically update the web page, you should select two seats just in case someone else submits their form before you do. Seats will be assigned on a first-come, first-served basis. Check the seating chart prior to the first class to avoid any confusion on the first night of class. Make specific attention to the FRONT and BACK references on the chart.

Don't forget to select your seat in the computer lab to complete the SEATING REQUEST form.

You also need to create a vCard and submit that to me as well.

If you cannot complete all of the questions immediately, click on the SAVE button to create a copy of this message in your DRAFT folder. You can then retrieve and complete it at a later time.
